All request for withdrawal and refunds must be filled out on this format below and submitted via e-mail to membership@saskatoonfieldlacrosse.ca
-All refunds are subject to a $50 Admin fee
-Refunds will not be given until jersey or other SFLA property is returned
-Refund requests will only be accepted up to and including the first game day of your child's league. No refund will be given after the first game day. For example; If your child plays 16U or 19U League you must submit this form on the first Sunday of May in order to qualify for your refund minus the $50 admin fee.
Please Note; If the registration was paid for using a credit card, the 3% fee will not be refunded.
Refunds will only be considered if received in writing detailing the following

Reason for requesting refund;
Would you like to be refunded by E-Transfer or back on your credit card.
